The protagonist of SMT Nocturne (known as Demi-Fiend in English version.) He was originally a high school student in modern-day Japan. Along with his friends Chiaki and Isamu, the protagonist was lured to the hospital by his class teacher Yuko Takao, and as a result was among those who survived after the Conception, which was the chaotic end of the old world that would await a new beginning.
To survive in the Vortex World, he was given a "gift" by the blond child - Magatama - the essence of demonic power which transformed into a half-human, half-demon being. His skin became riddled with black stripes akin to tattoos, and a horn appeared behind his nape that serves as an indication of his parasitic union with the Magatama. Hito-Shura gains the strength to fight with demons, earn powers from various Magatama, and to communicate and summon other demons that he manages to convince to fight alongside him.
As explained by the game creators, Hito-Shura means a being who bears demonic skin with human heart.

The first one who addressed him as Hito-Shura in the game was Futomimi. And then Hikawa - the one who triggered the Conception, noticed that from the Miroku Scripture, the author already predicted Hito-Shura will be born after the Conception and influence the result of re-creation. Thus every chosen human in the Vortex World tried to convince Hito-Shura to agree with their Reason / Kotowari, the concept that needed for creating the new world. He finally opened the path to Kagutsuchi. Depending on the decision made by player, he will either create a new world, return it to the original state before the Conception occurred, or destroy the metempsychosis of creation.
[edit] In Digital Devil Saga
In Digital Devil Saga, the Demi-Fiend makes an appearance in the Anahata underground dungeons as the ultimate challenge of the game. Upon entering the room where he is faced, the party is pulled into the Amala Network where they are confronted by the Demi-Fiend. He is accompanied by two other demons that will be replaced with different demons when they are defeated. Whenever two demons are defeated, the Demi-Fiend will use Gaia Rage, that will kill any character that is not asleep and set with the Null Sleep skill. Moreover, he will use Gaia Rage if any skills are set or spells used that null, reflect, or absorb any element. The demons summoned by him are Cu Chulainn, Girimehkala, Pixie, Arahabaki, Titania and Parvati.
In Digital Devil Saga 2, Satan will tell you that a heretic is trying to destroy the balance of the universe just like the protagonist of that game before the battle. It's highly suspected that he's speaking of the Demi-Fiend, because the Demi-fiend was the boss of the last game, but he could also mean Aleph from Shin Megami Tensei II, though it is unlikely because of the events of Shin Megami Tensei 3.

- The demon motif in the Protagonist's jacket sleeves are icons that were used to indicate demon numbers and status in the classic Megami Tensei games.
- Though the protagonist's personality is up to the player, for example Chiaki's insult around the beginning of the game, depending on your choice, express that you have had an ongoing interest in the occult magazines if you chose to defend bringing it to the hospital.
- During a New Cycle, you have the option of giving the Protagonist a leather jacket instead of the hooded jacket at the start. The costume change only lasts until the Conception, but the true effect of the new jacket is reflected in the status parameters, which will be slightly different than the base stats of the hooded jacket.
